Addison’s disease as suggested by hyperpigmentation, hyponatraemia and hypotension. A high 9 am plasma ACTH level with low or normal cortisol will confirm the diagnosis of primary hypoadrenalism. A low cortisol response with the short ACTH (synacthen) test would also show that the adrenal gland is not responding to ACTH.
